iQuestionnaire allows you to setup, take and report questionnaires in a self contained app. Its designed for people who wish to setup questionnaires then record an individuals responses in a face to face fashion. Results can be viewed on the iPad or exported as a CSV file for use in a spreadsheet or other applications.
